District builds and operates a client-centered integrated health management system, earning recognition for tailored health services
Geumcheon-gu announced that it received a commendation from the Minister of Health and Welfare after being named an outstanding institution at the 2026 Disability Health Management Project Performance Conference, held June 26.
The disability health management project is a national initiative aimed at systematically managing the health of people with disabilities in local communities and improving their quality of life by providing tailored health, medical and welfare services to guarantee their right to health.
Geumcheon-gu was recognized for building and operating a client-centered integrated health management system with the goal of reducing health disparities among people with disabilities and supporting their independent living in the community.
Particularly noteworthy was the district's work to establish a cooperative network centered on the Geumcheon-gu public health center, partnering with medical institutions, welfare centers and the district's association for sports for the disabled to deliver customized health services to residents with disabilities.
The district said the award confirmed external recognition of its sustained efforts to guarantee health rights for people with disabilities and its field-centered approach to project management, calling it an occasion that again demonstrated the strength of its community-based disability health management model.
In practice, Geumcheon-gu operates both home-visit and in-clinic services tailored to each client's health condition, providing individualized care that covers integrated home-visit health management, rehabilitation exercise programs, chronic disease management, health education, health screening referrals and welfare service referrals.
The district has also worked to identify health-vulnerable people with disabilities early and connect them with medical and welfare support systems, strengthening community-centered health care.
In addition, it has continuously expanded its community cooperation network to deliver seamless health services spanning prevention, rehabilitation and social participation.
"We will more tightly integrate health, medical and welfare services so that people with disabilities can lead healthy and safe lives in their communities," Geumcheon-gu District Mayor Yoo Sung-hoon said. "We will do our utmost to build Geumcheon into a healthy city where no one is left behind."
